Nell\u2019ultimo decennio la tecnica di ricostruzione delle immagini TC si \u00e8 basata su algoritmi di ricostruzione iterativi che hanno consentito una buona riduzione del rumore, ma un\u2019 eccessiva modifica della texture dell\u2019immagine con risultati\u00a0 definiti \u201cplastic like\u201d. Recentemente sono stati immessi nel mercato algoritmi di ricostruzione di immagini basati sul deep learning che utilizzano reti neurali convoluzionali profonde in grado di ridurre il rumore senza alterare la texture dell\u2019immagine. Questo ha determinato un aumento del rapporto segnale rumore (SNR) e rapporto contrasto rumore (CNR), che si traduce in una miglior qualit\u00e0\u0300 oggettiva dell\u2019immagine.<\/p><p>\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-35486e4 elementor-widget elementor-widget-text-editor\" data-id=\"35486e4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>Obiettivo:<\/strong><\/p><p>Lo scopo del lavoro \u00e8 stato quello di eseguire una valutazione tecnica confrontando le immagini ricostruite tramite algoritmi iterativi (IR) utilizzati nella routine clinica e la ricostruzione delle immagini con algoritmo deep learning (DL).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-d1543a1 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"d1543a1\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-6aada0a\" data-id=\"6aada0a\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-c4897de elementor-widget elementor-widget-text-editor\" data-id=\"c4897de\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>Materiali e metodi:<\/strong><\/p><p>Sono stati reclutati 38 pazienti (25 maschi) con BMI&lt;30. L\u2019esame TC e\u0300 stato svolto con scanner 64 strati, copertura 4 cm, l\u2019acquisizione in senso cranio-caudale \u00e8 stata eseguita con modalit\u00e0 retrospettiva.<\/p><p>Per ogni paziente sono state ricostruite immagini con tutti i livelli di intensit\u00e0\u0300 IR (10 lv) e DL (3 lv). La qualit\u00e0\u0300 oggettiva e\u0300 stata valutata attraverso l\u2019analisi del SNR e del CNR misurato mediante l\u2019utilizzo di quattro regioni d\u2019interesse (ROI) posizionate a livello del muscolo pettorale, dell\u2019aorta ascendente, della coronaria sinistra prossimale e del ramo prossimale della coronaria destra. Per evitare errori di posizionamento \u00e8 stata utilizzata la funzione \u201ccopia e incolla\u201d della workstation. Il rumore dell\u2019immagine \u00e8 stato definito come deviazione standard (SD) della ROI tracciata sul muscolo pettorale. La massima intensit\u00e0\u0300 di DL ha dimostrato una riduzione del rumore rispetto alla ricostruzione IR di media intensit\u00e0\u0300 utilizzata nella routine clinica con una media di valori DL 12.1 \u00b1 8.2 vs IR 19.2 \u00b1 25.9 permettendo cos\u00ec di ottenere un immagine meno rumorosa.<\/p><p><strong>\u00a0<\/strong><\/p><p>\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6077012 elementor-widget elementor-widget-image\" data-id=\"6077012\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div un miglioramento della qualit\u00e0\u0300 dell\u2019immagine ottenendo una riduzione del rumore del 37% e un conseguente miglioramento del SNR e CNR rispetto a IR di media intensit\u00e0. Inoltre, l&#8217;elevata qualit\u00e0 dell\u2019 immagine ottenuta tramite DL,<br \/>insieme all\u2019efficiente processo di ricostruzione ottenuto in un tempo \u2264 50s, apre la strada ad un possibile utilizzo nella routine clinica quotidiana. Il vantaggio che ne deriva \u00e8 quello di poter creare protocolli di acquisizione dedicati per le CCTA e sfruttare le capacit\u00e0 del DL di ricostruire l&#8217;alta qualit\u00e0 degli esami anche con una bassa dose. Inoltre, la migliore qualit\u00e0 dell\u2019immagine garantita dal DL aprirebbe la strada a protocolli di iniezione di mezzi di contrasto a volume ridotto, particolarmente utili negli anziani e nei pazienti con insufficienza cardiaca o con funzionalit\u00e0 renale compromessa. Pertanto, l&#8217;applicazione dell&#8217;algoritmo DL risulta utile nella pratica clinica per consentire una dose di radiazioni pi\u00f9 delicata senza effetti negativi sulla qualit\u00e0 dell&#8217;immagine e quindi sull&#8217;accuratezza diagnostica.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-50
