Caruso e del Prof. Laghi \u00e8 stata tra i primi in Italia ad utilizzare l\u2019algoritmo di intelligenza artificiale (AI) in Tomografia Computerizzata (TC) in fase di ricostruzione, mediante l\u2019implementazione della proposta di General Electric (GE). Il software di Deep Learning Reconstruction (DLR) True fidelity (TF) \u00e8 stato implementato per la prima volta sul territorio nazionale presso l\u2019Azienda Ospedaliero-Universitaria Sant\u2019Andrea di Roma (Polo Universit\u00e0 La Sapienza di Roma). L\u2019installazione dell\u2019algoritmo DLR ha fornito la possibilit\u00e0 di pubblicare un lavoro scientifico sullo stato dell\u2019arte nell\u2019utilizzo dell\u2019AI nella ricostruzione di immagini TC, per l\u2019ottimizzazione dei protocolli di acquisizione.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-685427f elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"685427f\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-c9518d0\" data-id=\"c9518d0\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-f793d44 elementor-widget elementor-widget-text-editor\" data-id=\"f793d44\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">L\u2019applicazione dell&#8217;intelligenza artificiale nella ricostruzione delle immagini TC nasce dall&#8217;esigenza costante di ridurre la dose di radiazione erogata al paziente, pur mantenendo una adeguata accuratezza diagnostica.<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">La prima generazione di ricostruzione iterativa (RI), introdotta a partire dal 2008, ha mostrato fin da subito alcuni limiti associati alla semeiotica delle immagini, ovvero l\u2019effetto noto come \u201cplastic like\u201d.<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">Le immagini ottenute dalle prime generazioni di RI, infatti, erano caratterizzate da una qualit\u00e0 di immagine differente se confrontate con quelle ricostruite con sistemi di retroproiezione filtrata (FBP), utilizzata in TC fin dai primi anni 70\u2019.<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">Le FBP permettono di ottenere una pi\u00f9 elevata confidenza in termini di risoluzione spaziale e di contrasto (edge enhancement), mentre la trama (texture) artificiale o l&#8217;aspetto macchiato delle RI di prima generazione riduceva la confidenza da parte del Medico Radiologo.<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">Se si considera invece l\u2019effetto plastificato iterativo (platic like), questo risulta essere pi\u00f9 evidente alle basse dosi: in questi casi infatti, per compensare il rumore quantico generato dalla bassa intensit\u00e0 di fotoni X, viene utilizzato un elevato livello di RI, con un conseguente spostamento della distribuzione del rumore verso le basse frequenze [1].\u00a0<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">Con l\u2019implementazione delle DLR in TC, sono stati superati i limiti sopra descritti, ottenendo una minore variazione nella texture delle immagini e una migliorata qualit\u00e0 di immagine, sia in termini di rapporto segnale-rumore (SNR-signal to noise ratio) che contrasto-rumore (CNR-contrast to noise ratio) [2-3].<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; font-size: 12pt;\"><span style=\"font-family: Arial, sans-serif;\">Attualmente, le immagini DLR mostrano una differente distribuzione del rumore quantico se confrontate mediante RI [4].<\/span><\/p><p class=\"MsoNormal\" style=\"margin: 0cm; Immagini ricostruite con FBP, ASiR-V 100% e DLIR-H con spessore di 2.5 mm l\u2019applicazione di Reti Neurali Convoluzionali Profonde (Deep Convolutional Neural Network), sottoinsieme del Deep Learning (Deep Learning Reconstruction). La rete neurale \u00e8 formata da milioni di nodi e ognuno di questi rappresenta una caratteristica\/parametro. Nel caso delle ricostruzioni DLR, le reti sono gi\u00e0 state &#8220;allenate&#8221; mediante immagini molto rumorose Vs. immagini FBP ad altissima qualit\u00e0, ovvero ad elevate dosi di radiazioni (tecnologia TF). La differenza tra i 2 dataset di immagini utilizzati per l\u2019allenamento del DLR viene utilizzata per calibrare (attivando o disattivando) i vari nodi. Quindi, nella pratica clinica, una volta acquisito lo slab di immagini, questo verr\u00e0 elaborato da un apposito ricostruttore che in pochi secondi applicher\u00e0 le opportune modifiche ai dati acquisiti, offrendo un\u2019immagine a bassa quantit\u00e0 di rumore.<\/p><p>L\u2019elevata capacit\u00e0 di denoising (riduzione del rumore) della tecnica DLR ha permesso un ulteriore riduzione della esposizione a dose radianti (Fig. 3), come evidenziato dalle prime pubblicazioni scientifiche [7-8-9].<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-3053490 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"3053490\" data-element_type=\"section\"
